Office Hours
Our business office is open Monday through Friday from 8:00 am to 4:00 pm.
All office visits are by appointment only. Appointments for all office
visits, diagnostic procedures and various outpatient hospital procedures
are scheduled by the

Kokomo and Lafayette are our satellite offices and may have hours that vary from week to week.
If you need to stop by to make a payment, drop off insurance papers or
schedule an appointment, please call (800) 582-9218 to verify what hours
the office will be open.
Payments and Insurance
We accept many insurance plans, but
please check with us if you have any questions about your eligibility. If you are covered by a HMO or PPO plan,
your co-payment is expected at the time of the office visit. If your plan requires a referral form from your primary care physician, it is your responsibility to obtain the referral form.
We are no longer a participating provider with Cigna Healthcare.
We do accept Medicare assignment, however all patients are responsible for the yearly deductible and co-insurance Medicare requires. As a service to our patients, we will file your insurance claims at no charge. If your insurance company does not pay the claim within 90 days, the claim is your responsibility and payment is expected.
Please feel free to ask us about your account at any time. If you are unable
to pay your entire bill immediately, our office manager will work out a payment
plan or make special arrangements to assist you in settling your account.
